The OMHS Diabetic Health Resource Center is located on the main floor of the Health Park , 1006 Ford Ave. It’s staffed by licensed nurses and dieticians who are certified diabetes educators, working with your physician to provide four your education needs.
Programs at the Diabetic Health Resource Center do require a physician’s referral and our staff can provide the assistance you need to arrange for a referral. All our programs are recognized by the American Diabetes Association for quality education, and most services are covered by private insurance, Medicare and Medicaid.
