Controlling Your Diabetes So It Doesn’t Control You

The OMHS Diabetes Health Resource Center is located on the main floor of the Health Park, 1006 Ford Ave.   It’s staffed by licensed nurses and dietitians who are certified diabetes educators, working with your physician to provide for your education needs.

Programs at the Diabetes Health Resource Center do require a physician’s referral and our staff can provide the assistance you need to arrange for a referral. All our programs are recognized by the American Diabetes Association for quality education and most services are covered by private insurance, Medicare and Medicaid.

In Balance Diabetes Education Program

The In Balance Diabetes Education Program directly impacts your quality of life.  The In Balance program covers the self-management skills patients with diabetes need to live a healthy and independent life.

The In Balance program provides outpatient services to include:

  • Glucose Monitor Instructions
  • Gestational Diabetes Program
  • Pre-Diabetes Education
  • Certified Insulin Pump Training
  • Training on Injectable Diabetes Medication

Our Self-Care Education includes:

  • Increasing Physical Activity
  • Managing Blood Glucose
  • Preventing Long-Term Complications
  • Changing Behavior and Managing Stress
